The education exhibit 0131CE-e of Paul Zoumpoulis, Stergios Prapavesis et al on the subject of implementation of local anesthesia duting TRUS-guided prostate biopsy was awarded the Certificate of Merit during the RSNA 2003 Radiology Congress in Chicago, USA.
Based on an extremely large number of cases, the exhibit examined in detail the technique, the advantages, the practical advantages and the possible complications of implementation of local anesthesia during prostate biopsy. This refinement in biopsy technique allows for an increased number of biopsy cores to be sampled, with less patient discomfort and ensures better patient cooperation during the biopsy procedure.
Local anesthesia before TRUS-guided prostate biopsy: Technical results and side effects.

Learning objectives:
- To demonstrate the procedure of TRUS guided local anesthesia before prostate biopsy.
- To estimate the technical results, the efficacy and the side effects of TRUS guided local anesthesia before prostate biopsy.
During the period 2000-2002, 1200 US-guided prostate biopsies have been performed following local periprostatic anesthesia, which was performed by injecting 8-15 cc of Xylocaine and 2% adrenaline through the biopsy needle. The technique, the consequences on the US image and the complications and side effects were compared to 4500 US-guided biopsies performed between the years 1994-2002, without local anesthesia.
Local anesthesia performed prior to TRUS-guided prostate biopsy is an efficient way to avoid pain during the procedure. There was no significant difference between the number and severity of complications in the group with local anesthesia compared to the group without anesthesia. There were no significant side effects caused by the infusion of the anesthesia drug. However the infusion the drug may cause a degradation of the TRUS image, resulting in a degree of difficulty guiding the needle.
Conclusion:
Our conclusion is that US guided local anesthesia before TRUS biopsy is useful for assuring cooperation of the patient during this often painful procedure, and essential if TRUS findings call for a large (>10) number of biopsies.
